Astell House, as a residential home, is unusual in that it has two nurses as part of the care team.  The nurses and their expertise to understanding each resident’s clinical care as well as getting know them personally, responding to any change in condition rapidly.

We work closely alongside residents and their families to create a person centred plan of care for each individual, which is regularly reviewed. Each resident has a named key worker who helps them settle in and becomes their main contact to help with day to day needs. We offer specialist care for those with dementia in all its varying forms.

The physiotherapist visits Astell House three times a week to give assessments and treatment sessions, supporting rehabilitation after a hospital stay or illness, working to prevent falls, supporting people’s mobility and independence.

We liaise closely with a multi-disciplinary team of external health professionals including doctors and specialist nurses to ensure every resident’s care needs are met. We have close links with our local GP surgery who hold a fortnightly surgery at Astell house and we have other visiting services such as chiropodists and opticians. Our team liaises with the NHS rapid response team to avoid unnecessary hospital admissions.
